As a patient of Merrindale Medical Centre, you have the right to:

Privacy and confidentiality of your personal and health information
Access your medical records in accordance with privacy legislation

Request an interpreter or support person where appropriate
Your Responsibilities as a Patient
To help us provide the best possible care, we ask patients to:
Treat staff and other patients with courtesy and respect
Provide accurate and complete information about your health and medical history
Inform the practice of any changes to your contact details
Follow agreed treatment plans or discuss concerns with your doctor
Attend appointments on time and notify the practice if you need to cancel
Understand that emergencies and complex medical issues may occasionally result in delays
Respect the privacy and confidentiality of other patients and staff
Refrain from abusive, aggressive, threatening, or inappropriate behaviour towards staff or other patients
Merrindale Medical Centre has a zero-tolerance policy towards abusive or violent behaviour.
